Margolin, Keinarth & Alberda, M.D. have been approved by the Texas Department of State Health Services (DSHS) to administer the COVID-19 vaccine. The brand Moderna will be administered. https://www.modernatx.com/ 

We will provide Covid-19 vaccinations to our patients in accordance with the Texas DSHS Covid-19 vaccine allocation and prioritization guidelines. Phase 1A is allocated to first responders and healthcare workers and they will be receiving our limited supply first. Then we will go into phase 1B, which is includes: People 65 years of age and older, people 16 years of age and older with at least one chronic medical condition that puts them at increased risk for severe illness from the virus that causes COVID-19, such as but not limited to: Cancer, Chronic kidney disease, COPD (chronic obstructive pulmonary disease), Heart conditions, such as heart failure, coronary artery disease or cardiomyopathies, Solid-organ transplantation, Obesity and severe obesity (body mass index of 30 kg/m2 or higher), Pregnancy, Sickle cell disease and Type 2 diabetes mellitus.

 Future shipments have not been determined by the state yet. When our office knows when we will be receiving a shipment for our remaining patient population, someone from our office will then contact you to schedule an appointment if you have requested to be placed on the COVID-19 vaccine list. 

We are kindly requesting that you PLEASE DO NOT CALL the clinic to request the Moderna vaccine or to be placed on the vaccine list, and/or to call to get updates. It is important to keep our phone lines available to patients who are experiencing acute care needs. To request to be placed on the COVID-19 Moderna Vaccine List fill out this request form. Our triage nurses will be reviewing the criteria and will be contacting you if you meet the Texas priority guidelines.  Vaccines are limited state wide. Therefore, you may not receive an immediate response from Margolin, Keinarth, and Alberda, M.D. However, please remain patient and we will contact you as we obtain more vaccines. PLEASE make sure if you are placed on the vaccine schedule that you keep the appointment. We will update our website for you to view the most up-to-date information on the Moderna Vaccine that will be available in our clinic soon.

If you are eligible to get a COVID vaccine at another facility before it is available at our clinic, we encourage you to do so. We are committed to keeping our patients safe and vaccinated and we appreciate your patience during this difficult time.
